*CAUTION* People can be seriously hurt with this prank. Don't do this if you are intending to hurt someone because it can do some serious damage if they crack their head open on the toilet bowl or something. *CAUTION*
...with that said...
Wait for your victim to be away for some time, then find a tiled surface that they would walk on, like the entryway to an apartment. Ideally you do not want to be around when they come in and fall on their ass, as they will likely be pissed, so I suggest you set up a webcam or camera to record the action and either leave entirely or hide in your room.
Now, to force one of these slips to happen, you can set up a home phone across a tiled floor, for example in a kitchen. Now, you ring the phone yourself when you know only they are home, and watch remotely from your webcam as they run to get the phone and totally biff it on the floor!
